In-hospital triage is performed by highly skilled nurses called triage nurses. JTAS (Urgency Assessment Support System) is a tool that objectively and systematically supports this assessment.

[About revision]
JTAS (Japan Triage and Acuity Scale) was developed in 2012 based on CTAS (Canadian Triage and Acuity Scale), an "emergency patient emergency determination support system" developed in Canada. It was adopted as a standard indicator for in-hospital triage as a requirement for calculating medical fees at the time, and many medical professionals conducted educational activities to popularize it. Subsequently, in consideration of changes in social conditions and the medical system, we revised the guidelines and published JTAS2017 in 2017.
JTAS has become widely used in various medical institutions, but the novel coronavirus pandemic that began in 2019 has had a major impact on medical care, disrupting the balance between supply and demand. This was also a valuable opportunity to reconsider how to determine the degree of urgency.
Taking this situation into consideration, this JTAS2023 has added new topics such as emerging infectious diseases (COVID-19), the elderly, situations where brain death can be considered, abuse, and radiation exposure. JTAS2023 is entering a new phase, and the emergency determination support system of Japan, which is one of the most aging countries in the world, is required to function as a social safety net.

[Overview of JTAS2023]
The triage nurse can assign an emergency level to each patient based on the JTAS, keeping in mind the patient's appearance (skin tone, degree of injury, etc.), objective findings such as vital signs, and information on the health condition up until the hospital visit. can. The JTAS2023 symptom list at the time of visit consists of 17 items including 168 symptoms. This symptom list has the same arrangement for adults and children, and can be used differently depending on the age of the patient visiting the hospital outpatient department to determine the degree of emergency. The symptom list at the time of visit standardizes the symptoms at the time of visit or the reason for visit. There are five levels of urgency: level 1 is resuscitation, level 2 is emergency, level 3 is semi-urgent, level 4 is low emergency, and level 5 is non-urgent. However, please keep in mind that determining urgency is a process of patient evaluation and is not like a scale on a ruler.
