Modivcare members can book and manage trips.
The Modivcare app is an easy, convenient and accessible way to schedule and manage all of your Modivcare non-emergency transportation needs. No need to call a care center.
For eligible members who drive themselves to appointments, they can submit their claims for Mileage Reimbursement.
Existing Member Services Website accounts will not work logging in.

it would be nice if on the very first section of the app you would say what all you need that I could have it ready because filling this out I had to call the social worker to get a bunch of information but I did not know as for courts address and and all of that that it if it was already noted in the beginning to say this is what you need need then it would have been much faster and easier thank you

I'm only giving it two you cuz I actually exist... I truly do not know what happened to this app when it started off it somewhat primitive but special to have, I really don't know what happened but each year each month each hour each day it gets worse ‼️. Anthem Blue Cross you've got to find better....

Update: somehow it has developed a new glitch. Now the signature box will only let you draw lines. Please fix
