The patient profiles are funny as hell. It's good for a bit,but you run out of tries then have to wait and do nothing or watch a few commercials. That's pretty much forcing you to watch or waste time doing literally nothing. Fix that and I'll reevaluate.

This game is behind a paywall, you can go as far as you run out of cash, which is probably 10-15 mins, then you have to buy in game cash with real cash, this will be really expensive to do that.
