With the free Horizon NJ MyCare app, Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members can:
• View covered benefits, including medical, pharmacy, behavioral health, dental, vision and more
• View, download or request a new member ID card
• Change your PCP
• Find a doctor, dentist or other health care provider
• View your Care Manager, if you have one
• Find community support and resources available to you
• Learn about additional services available to Horizon NJ TotalCare (HMO D-SNP) members only such as:
➢ Horizon EXTRA Benefits Card features
➢ Telemedicine
➢ And more!

less service options than full website or mobile web page, can't do a specialist search either. basic membership info and benefits with option to contact support which is helpful at least. it's functional.
Not the best. Can't change PCP on the app. Horrible navigation as well.
The app doesn't let me sign in. I've changed my password several times and when I get to verifying my identity, it always says, "we're experiencing technical difficulties." When is this going to be fixed?
App has not worked in over a month. Dragging your feet fixing this with no response for over a month, meanwhile every day it's broken you're limiting people's access to convenient healthcare.
